New Patient Registration Packet for Female Patients * For Male Patients
Print and complete this form BEFORE your initial appointment. The Registration packet include a comprehensive medical history. 

Medical Pre-op Packet
This packet includes General Nutritional Guidelines, the Phase Diet Instructions and discharge instruction. This packet will be reviewed with you during your pre-op medical consultation 2-weeks before surgery.

Pre-Operative Written Exam For Bariatric Surgery
This is an educational tool for our patients. Many patients have told us that they have learned more from taking this examination than from all their previous research and reading. The exam MUST be completed before your pre-op medical appointment. The Bariatric Internists will review your exam with you.

Tax Break Information
The American government has recognized the need for physician-directed weight loss and bariatric surgery in the medical treatment of obesity. You may qualify for an income tax deduction based on out-of-pocket expenses for medically directed weight loss.
